Description

If you're an employee, use form SC5 to apply to your employer for Ordinary Statutory Paternity Pay (OSPP) and/or Ordinary paternity leave if you are becoming an adoptive parent and the child is being adopted from abroad. The form explains the terms and conditions that apply to OSPP, and asks for some basic details about the adoption.
