Use form CA1559 as an annuity certificate for a Salary-Related pension scheme or a Salary-Relate part of a Mixed Benefit scheme. When an occupational pension scheme ceases to be a Contracted-out scheme we may approve arrangements for the securing of Guaranteed Minimum Pensions (GMPs) and/or post' 97 COSR rights.
